That, at least, has been the experience with clubs I've been associated with. The referees simply do not allow such thing to occur more than once.
I can't tell from experience, but I've seen it in discussions, interviews, and the press, that the English Premier League plays a much fiercer brand of football than elsewhere, so my guess is that an English referee would tolerate more roughness than one from somewhere else might have. On the other hand, German referees are criticized for being too harsh. There doesn't seem to be a middle ground on this.
One of the most respected German referees was Dr. Markus Merk, who officiated for years at top level. By profession he was a dentist. Gianluigi Collina (I think that's right) was an Italian referee who was equally respected. Neither of them would have allowed what happened Sunday night.

When 'defunct' behaviour pays off, people are acting accordingly. (We can see this also here in the forum...) And of course the players realized very early in the match that it pays off. Clearly the fault of the referee.
